Home Visit Assessments

We come to you. Look at what's working, what's getting harder, what you're worried about. Then we figure out solutions together. You'll get a written summary covering immediate needs, equipment recommendations, and planning for what's ahead. No jargon, just clear information you can use.

EQUIPMENT ASSESSMENT

We assess what you need, help with funding applications, and coordinate the whole process. We know which suppliers are good, which equipment actually works, and how to navigate funding systems. We handle the bureaucracy so you don't have to.

CARE PLANNING

As things change, your plan changes. We work with you to stay ahead of needs without dwelling on worst-case scenarios. This includes equipment timing (what you need now vs in three months), coordinating with other health professionals, and adjusting strategies as your function changes.


END OF LIFE GOALS

Got something you're determined to do? We help work out if it's possible and how to make it happen safely. Last family gathering. One more camping trip. Attending a wedding. Seeing the garden in spring. Whatever matters to you, we problem-solve the logistics, equipment, symptom management, and risk.

DYING AT HOME SUPPORT

For people who want to die at home, we provide practical planning—what equipment you'll need, how to manage comfort, what support your family will require. We talk honestly about what's involved, help you prepare the physical environment, coordinate equipment delivery, and work with your palliative care team to make it possible.

FAMILY AND CARER SUPPORT

Supporting someone at end of life is complex. We provide practical education and strategies for families and informal carers. This includes showing you how to use equipment safely, discussing what to expect as things change, and working through concerns about providing care at home. We help you feel more confident in supporting the person you care for.

GETTING STARTED

Contact us directly, ask your support coordinator to refer you, or have your GP or palliative care specialist send a referral.

We'll have a conversation about your situation, what you're hoping for, and whether we're the right fit. We respond promptly to referrals and enquiries, and we'll work out together what timing makes sense for you.

FIRST HOME VISIT

We spend proper time understanding your situation

With your carer and/or family present, we talk about what you're managing, what's getting harder, your home setup, equipment needs, goals, and worries about what's ahead.

You'll get a written summary including current situation and functional status, equipment recommendations (immediate and future needs), care planning for progressive decline and next steps.

Copies go to you, your family (if you want), and your referrers or coordinators.

ONGOING SUPPORT

Follow-up visits as your needs change.

Could be weekly, monthly, or as-needed depending on your situation.

Equipment reviews when things aren't working or your function changes.

Care plan updates as your condition progresses.

Problem-solving visits when new challenges come up or goals change.

Coordination with your palliative care team, GP, other allied health professionals

Private Funding For people who prefer to self-fund their care. Flexible service delivery tailored to your needs.

Support At Home If you're self-managing your package or working with a package coordinator, you can use funding for specialised palliative OT services.

Service Areas

Melbourne metropolitan area. Home visiting service—we come to you.

What if we're not sure what we need? That's what the first visit is for. We help you identify needs you might not have thought about yet.

Do you work with our existing services? Yes. We coordinate with your palliative care team, GP, specialists, and other therapists. We add to your support, not duplicate it.

What if things change quickly? We can arrange additional visits or coordinate with your palliative care team for urgent support.

What about weekends or after hours? Standard visits are business hours. We can discuss flexible timing for specific goals or events where needed.

How long are visits? First visit is around 90 minutes. Follow-up visits are usually 45-60 minutes, though complex needs can mean longer visits. We tailor the time to what you need.
